Aged Care Case Manager
We are currently looking for an Aged Care Case Manager for a newly registered Aged Care provider.
The business has all required systems, policies & procedures, staff and documentation but requires experience to assist with launching, initial set up, client attraction and on-boarding of clients.

About the Role:
As the Aged Care Case Manager, you will be integral in the launch and initial operations with responsibilities including:
  • Review of business documentation, policies & procedures and final set up advise
  • Business development and client attraction
  • Initial client assessment, planning, agreements and on-boarding
  • Develop, implement, evaluate, and regularly review client-cantered care plans
  • Work closely with roster team to coordinate the delivery of quality care
  • Oversee the financial management of care planning to ensure services are delivered according to legislation and within allocated budget
  • Maintain up to date client records of assessments, care plans and services
  • Be involved with client changes and incident reports
  • Providing advice and expertise on ways to improve services and quality
About you:
  • 5+ years Case Management experience (experience with start-up or small providers highly regarded)
  • Clinical background of AIN, EN, RN or Allied health highly regarded
  • A thorough knowledge of Aged Care standards and legislation
  • Prior experience with signing up and on-boarding clients
  • Proficiency in conducting assessments, care planning, implementation, evaluation, and documentation of quality client care in line with Consumer Directed Care principles
  • Strong communication skills and ability to build rapport with clients from various backgrounds
Essential requirements:
  • Current police check
  • Working With Children Check or willingness to obtain
  • Current First Aid and CPR
  • Up to date Flu and COVID-19 Vaccinations
  • Unrestricted Australian working rights
  • Full Drivers License and reliable vehicle
