Our client is seeking an experienced Disability Support Coordinator to join their dedicated team.
In this role, you will work closely with NDIS participants, coordinating and implementing supports funded through their NDIS plans.
Your primary focus will be to ensure participants achieve their expected goals related to independence, well-being, and community participation.

$75000- $90000 plus Super 

Key Responsibilities:


  • NDIS Plan Coordination:
    • Monitor and implement all supports outlined in participants’ funded NDIS plans.
    • Ensure that all funded supports are effectively managed and delivered.
  • Participant Support:
    • Collaborate with participants to identify goals and develop strategies to achieve them.
    • Provide ongoing support to enhance participants’ independence and well-being.
  • Collaboration and Communication:
    • Work as part of a passionate, supportive, and flexible team that values collaboration.
    • Communicate effectively with participants, families, and service providers to ensure seamless service delivery.
  • Progress Monitoring:
    • Regularly review participants’ progress against their goals and adjust support plans as needed.
    • Ensure compliance with NDIS guidelines and quality standards.

Qualifications and Experience:


  • Proven experience in disability support coordination, preferably within the NDIS framework.
  • Strong understanding of NDIS funding and planning processes.
  • Excellent communication, organizational, and problem-solving skills.
  • A commitment to promoting independence and enhancing the quality of life for NDIS participants.

What you need


  • Experience working with NDIS participants with knowledge of NDIS Operational Guidelines.
  • Qualification in Disability Services, Aged Care or other relevant Community Services
  • Minimum 2-year experience as a Disability Support Coordinator.
  • Highly developed communication and interpersonal skills.
  • Team player who is driven by compassion and role purpose.
  • An Australian driver's license, Working with Children Check, National Police Clearance, First Aid Certificate and a NDIS Workers Screening Check.
  • Own vehicle and comprehensive car insurance.
  • Psychosocial Recover Training (desirable)
